Accuracy
Accuracy in osu! is how close to perfect your hits were on a beatmap, expressed as a percentage. Every note has a perfect-hit window; how much of that window you used translates to a hit-judgement (300, 100, 50, or miss in osu!standard) and accuracy is the weighted ratio of those judgements. Accuracy is one of the inputs to a play's pp value — higher accuracy on the same map and combo earns more pp.
Per-mode differences
Accuracy formulas differ between osu!standard, taiko, catch, and mania. The high-level idea is the same — better hits give higher accuracy — but the exact weights vary.
Profile accuracy
The accuracy shown on your profile is a weighted average across your top plays, not a simple mean of every play you've made.
